    Why the Coalition?

    With hair loss treatments often being marketed without regulation or accountability, hair loss sufferers need credible information about legitimate treatments and the ethical physicians who provide them.

    The Coalition of Independent Hair Restoration Physicians is a carefully selected group of skilled and ethical physicians who are united in providing patients with outstanding education and results.

    Physician Accountability

    Membership in the Coalition requires members to maintain a high degree of patient satisfaction. Members care for their patients knowing that their patients are free to post their experiences on the popular Hair Restoration Discussion Forum. On this open forum patients are free to critique physicians and to hold them accountable for their actions. This forum is run and moderated by hair restoration patients.

    In such a transparent environment our members are held publicly accountable for their actions and results. Members who fail to meet the high technical and ethical membership standards set by the Coalition are removed from the group.

    However, we ask that patients first give their physician the opportunity to satisfy their concerns privately before posting any public criticisms on the discussion forum.

    Membership Standards

    The skill, technique and talent of hair restoration physicians vary widely, as do the end results. Only a minority of leading physicians provide truly state of the art hair restoration.

    Coalition members are carefully reviewed and granted membership based strictly on their high level of integrity, skill, experience and ability to perform ultra refined follicular unit transplantation.

    All Coalition members have made the investment in time and resources to perform a highly refined and difficult to perform state of the art procedure called “Ultra Refined Follicular Unit Hair Transplantation”.

    While many surgeons now perform standard follicular unit grafting, only a minority have risen to the challenge of using very tiny incisions and grafts to achieve ultra refined results. This ultra refined procedure, while very demanding on the physician and staff, produces excellent results for the patient with optimal density and rapid healing. It also allows the patient to achieve their goal for fullness and density with a minimal number of surgical sessions.

    A physician must meet the following requirements before being granted membership in the Coalition of Independent Hair Restoration Physicians:

    • A demonstrated capability to successfully perform large sessions of ultra refined follicular unit grafting using tiny incisions and grafts that are microscopically prepared.
    • The ability to dense pack tiny follicular unit grafts in a given area when appropriate.
    • Mastery and control of the orientation and direction of the transplanted grafts to achieve a completely natural appearance.
    • Dedication to doing state of the art hair restoration exclusively or at least as the main specialty of the clinic.
    • Excellent patient results demonstrating a high level of artistry and naturalness throughout, as well as excellent growth rates.
    • An excellent reputation of the physician amongst colleagues and former patients.
    • Several years experience in performing hair restoration surgery.
    • Medical training, depth of background, board certifications, honors, credentials, lectures, and published articles and books.

    Membership Selection Process

    Physicians with a proven track record of providing excellent ultra refined follicular unit hair transplantation and who meet the Coalition’s membership criteria will be considered for membership.

    In addition to reviewing the applicant physician’s credentials, technique and results, input from patients and current members of the Coalition will be considered prior to membership being granted.

     

  • Future Hair Loss Treatments

    Some day, perhaps in the distant future, baldness may become a curable aliment. Scientists are currently researching gene therapies to alter a person's genetic vulnerability to hair loss. However, the actual implementation of such hair loss treatments may be decades away.

    In the near future, drug treatments which inhibit the balding process seem more promising. The hair loss drug Propecia (finasteride) has been proven successful in slowing and/or stopping hair loss by blocking the conversion of testosterone into dihydrotestosterone (DHT) in the scalp.

    Another promising drug that inhibits the development of hair loss causing DHT hormones is Dutasteride.

    Like Propecia, it also inhibits the creation of DHT. And based on preliminary clinical studies, dutasteride promises to be even more successful than Propecia in inhibiting the production of DHT.

    In addition, Dutasteride inhibits the activities of two types of 5-alpha-reductase enzymes. In contrast, Propecia (Finasteride) only inhibits one type. Dutasteride has been shown to decrease levels of DHT by 90% after only two weeks, making it a more powerful and faster-acting weapon against hair loss than Propecia (Finasteride).

    Dutasteride is not yet FDA approved for the treatment of hair loss. However, it is being marketed as Avodart by GlaxoSmithKline as a hair loss treatment. Some hair restoration physicians also prescribe it for the treatment of hair loss. Like Propecia, Avodart is not safe for women and children.

    In time, perhaps even more effective hair loss drugs will be developed to inhibit the hair loss process.

    Future Hair Transplants - an endless supply of bald resistant donor hairs?

    The amount of hair you can transplant is ultimately limited by the amount of hair follicles you can safely relocate from the bald resistant donor area at the back and sides of your head.

    Some day it may be possible for physicians to create multiple hair follicles from one original follicle. This process typically called "hair cloning" or more correctly "Hair Multiplication" is currently being investigated by several research scientists and hair restoration physicians.

    If and when this process is successfully developed patients would no longer be limited by the finite amount of bald resistant hairs that can be relocated from their donor area. Hair multiplication would result in a virtually limitless supply of hair available for hair transplantation.

    This would be especially good news for men or women with extensive baldness and a very limited supply of donor hair. Even those with extensive baldness would theoretically be able achieve thick full heads of hair.

    However, experts believe that we are at least ten years away from hair multiplication being available, if even then.

    For most men and women, hair loss can be effectively halted or even reversed with current hair loss treatments. But the future holds the promise of even more effective treatments and perhaps some day even a "hair loss cure".

    It is important to have realistic expectations and to find a hair transplant surgeon who can meet or exceed your expectations.

    Surgical hair restoration is largely a matter of "Supply and Demand".

    The look you can and will achieve with hair transplants is determined by several factors. But the most critical factor is really a question of "supply and demand".

     

    The supply of hair is how much viable bald resistant donor hair you have. The demand is how much bald area you want to cover and to what degree of thickness.

    Your “Donor Hair” determines what you can achieve.

    For virtually all hair loss sufferers, even those with severe baldness, there is typically hair that grows for life on the sides and back of the head. This is because the hair in this "donor area" is genetically different from the hair on the top of the head in people who go bald.

    The hair follicles in the "donor area" are genetically resistant to the effects of such baldness causing hormones as (DHT) dihydrotestosterone. That's why they continue to grow, while the hair on the top (in the male pattern baldness area) thins and goes bald over time.

    How much viable donor hair you have will largely determine how much you can accomplish with hair transplants. Most people have enough viable donor hair to do at least one or two hair transplant procedures.

    What is Traction Alopecia?

    Traction alopecia is a form of hair loss resulting from the repeated yet unintentional pulling, twisting or rubbing of the hair. Traction alopecia can affect both men and women, though it's more commonly seen in women. Though it can be temporary, balding from traction alopecia may become permanent if these traumatic forces are applied to the hair and scalp repeatedly and for prolonged periods, resulting in scarring.

    What Causes Traction Alopecia?

    Practices that may result in traction alopecia include; wearing tight braids, ponytails or cornrows and extended use of hair extensions.  Due to the popularity of these hairstyles, traction alopecia has become one of the leading causes of hair loss among African America women.

    Balding from traction alopecia can also occur from repeatedly wearing tight-fitting hats, helmets or other headgear.This is likely where the popular hair loss myth about hats causing hair loss derived, but it is important to note that, under normal circumstances, wearing a properly fitted hat will not negatively impact hair growth.

    Treating Traction Alopecia

    The first step in treating traction alopeciais to change the habits leading to the condition. This may mean adopting a new hairstyle or altering your wardrobe. Finally, applying a vaso dilator like Rogaine(minoxidil) to the affected area can increase blood flow to the damaged follicles and effectively speed up new hair growth. Assuming the hair follicles have not been permanently damaged, new hair growth will likely occur though it may take several months.

    In cases of severe and permanent tractional opecia, the only effective method of restoring hair to the bald areas is surgical hair restoration.   • Progesterone

    This hair loss product, meant for women, is a synthetic form of Progesterone, a hormone and steroid that occurs naturally in the ovaries. The body uses the Progesterone treatment to control a variety of functions. These include the regulating of the menstrual cycles, anticancer properties, the slowing and prevention of bone loss and ensuring the female sexual organs’ health. Another important function of Progesterone is to maintain the hormone balance of estrogen.

    But in regards to hair loss, Progesterone treatment is an effective DHT-inhibitor. However, there has not been much research conducted as to Progesterone’s impact to male and female pattern baldness (MPB, FPB). What research was conducted showed that it may be effective at stopping hair loss, but not at hair regrowth.

    Nevertheless, topical Progesterone has been used by some European dermatologists in Europe to treat MPB and FPB. For this, they only used 1% or 2% topical concentrations, and no more than 40mg a day. Researchers believe that higher doses could cause menstrual irregularities.

    Topical Progesterone is available as capsules, vaginal creams and suppositories. Patients use the medication by rubbing it into the body’s soft areas, such as the breast, abdomen, inner thighs and the palms of the hands (which is thought to be the most effective area).

    It’s advised that women use about 1/8-1/2 teaspoon, once to twice daily, for 21days. Physicians also advise women to stop using the medication for 5-7 days each month. They also advise women to change the application areas periodically.

    Like other aerosol products, ProTHIK is only recommended for areas of hair loss 1.5 - 1 inches posterior to the frontal hairline however, it's considered especially effective in the vertex (crown) region.  Despite the more specific application, ProTHIK remains popular with many consumers because of its overall effective nature, natural appearance, and lasting durability.  On the flip side, some ProTHIK users have expressed concerns about messy application and clogging.

This is more than 20% higher than the liquid 5% minoxidil solution at 64%. Rogaine foam also comes without the inconveniences of its liquid counterpart such as scalp irritation, flaking, and messy application. The foam dries almost instantly as opposed to the liquid's 15 minute waiting period.

    Note: Despite what the insert says, Rogaine works just as well for frontal hair loss, recession, and thinning hair.

    How It Works:

    Rogaine foam uses versafoam technology to aid the delivery of the minoxidil to the hair follicles. As the minoxidil absorbs into the scalp, it stimulates follicles, revitalizing them, promoting thicker and healthier hair growth. This promotes a thicker and fuller appearance of hair.

    Developing Realistic Expectations:

    While Rogaine foam is supreme in many ways, it's unlikely to regrow thick terminal hair in completely bald areas. For best results, men ought to consider combining Rogaine with Propecia (finasteride), the only FDA approved and clinically proven DHT inhibitor to stop hair loss.     Don't expect immediate results. Just as hair loss is a slow process, regrowing it is too. Thus, it's recommended to use Rogaine for approximately 6 to 12 months before determining its effectiveness. Documenting your progress with bi-monthly photos can help you better evaluate the results.

    The quest for a hair loss “cure”

    It was Hippocrates, the “Father of Modern Medicine”, who first recognized a connection between hair loss and the sexual organs. Hippocrates’ studies of eunuchs (men castrated before puberty) observed that these men did not suffer from hair loss.

    We know today that this is due to the absence of testosterone, which is normally converted into dyhydrotestosterone or DHT – the active ingredient in baldness. In March 1995, Duke University researchers came to the conclusion that " . . . while castration may be a cure, it is not commercially acceptable."

    Hippocrates had a personal interest in finding a cure for baldness as he suffered from hair loss. He developed a number of different treatments including a mixture of horseradish, cumin, pigeon droppings, and nettles to the scalp. This and other treatments failed to work and he lost the rest of his hair. The area of permanent hair on the back and sides of the head (from which donor hair is taken for hair transplants) is also known as the “Hippocratic wreath.”
